About ACIL
The Appalachian Center for Independent Living (ACIL) is a nonprofit organization dedicated to empowering people with disabilities to live independently and participate fully in their communities. Since 1982, ACIL has provided advocacy, skills training, peer support, and transition services across West Virginia.
Through the Community Living Services Program (CLSP), ACIL assists individuals with disabilities in maintaining safe, accessible, and independent living environments. The program provides assistive devices and home modifications—such as ramps, grab bars, and bathroom adaptations—to help prevent institutionalization and promote full participation in community life.
Position Summary
The CLSP Coordinator manages all aspects of the Community Living Services Program, including eligibility screening, needs assessments, coordination of home modifications and assistive technology, and collaboration with contractors and funding partners.
The ideal candidate is detail-oriented, resourceful, and committed to helping individuals with disabilities remain in their homes safely and independently.
Key Responsibilities
- Receive and review program referrals; determine eligibility and priority of need based on program guidelines.
- Conduct in-home needs assessments to evaluate accessibility barriers and identify appropriate modifications or devices.
- Develop individualized service plans with the consumer that outline project needs, estimated costs, and intended outcomes.
- Coordinate with licensed contractors, vendors, and funding sources to ensure timely and compliant completion of projects.
- Maintain accurate program files, documentation, and progress reports in accordance with grant and agency requirements.
- Conduct follow-up evaluations to ensure consumer satisfaction and project effectiveness.
- In cooperation with the Executive Director, track and report program expenditures and assist in preparing reports for state and federal funders.
- Develop and maintain a network of community partners and contractors for ongoing program collaboration.
- Collaborate with ACIL staff to connect consumers with additional Core and Plus Services as needed for wraparound support (e.g., transportation, skills training, advocacy, transition support, and peer mentoring).
- Participate in ACIL outreach, community education, and accessibility awareness activities.
Qualifications
- A degree in Human Services, Rehabilitation, Social Work, or related field. Equivalent experience in the management of construction and maintenance projects will be considered.
- 2 years of experience in case management, housing accessibility, or community-based services.
- Strong communication, organizational, and problem-solving skills.
- Ability to manage multiple projects, deadlines, and documentation requirements.
- Working knowledge of Microsoft Office and database recordkeeping.
- Valid driver’s license and reliable transportation required.
Preferred Qualifications
- ADA Coordinator Certification is preferred, but experience coordinating home modification or accessibility projects is acceptable.
- Knowledge of assistive technology and adaptive equipment.
- Familiarity with federal and state disability service programs.
- Knowledge of the Independent Living philosophy
- Familiarity with the Americans with Disabilities Act (ADA), Section 504 of the Rehabilitation Act of 1973, and the Fair Housing Act.
